SUMMER July - September 2007

Dear patients, friends and colleagues,
We are pleased to announce there opening of Skinfo® on the ground floor of the CityPark Complex, making Skinfo® even more accessible for all your needs. Every product that is presented in Skinfo® has been looked over very closely, compared to similar products in the industry, and chosen for their uniqueness and outstanding qualities. With thousands of products available, we hand-select based on formulation, efficacy and cosmetic elegance, now carrying over 400 elite products.

On June 21st, we held our annual staff training day. We presented on medical and cosmetic dermatology, both new trends and higher education. In our training we also discussed past andc urrent research projects. This year we were delighted to have Trudy Shelley, Sr. Allergan Practice Consultant, present on marketing trends in dermatology.

Unfortunately, Sandra Elkhouly, PA-C is leaving our practice in late September. She has decided to relocate to her home town of Tampa, FL after her recent marriage. We will miss her terribly, but of course we are sending her off withour very best wishes for success andhappiness in the future. We are currently interviewing potential PA candidates and hope to have an outstanding replacement for Sandra when she leaves.

Lastly, we have added three new treatment options to our menu. Perlane, finally FDA approved, a very robust injectable filler lasting about 9-15 months and producing excellent volume; the Pearl, a treatment device that improves facial sun damage and aging changes (such as brownspots, wrinkles, etc) bygently removing thetop layer of skin; and Cosmelan® Depigmentation Treatment, a peel designed to reduce melasma in 1 treatment and recommended for all skin types. We are very excited to be able to offer to you these new technologies! See Aesthetic Dermatology for more information.

Thank you to everyone who bought sunscreen in May. Previously announced, we donated 5% of the proceeds from sunscreen sales to the Skin Cance Foundation totaling over $800.

Have a great summer!
Amy Forman Taub, MD

Different Fillers There are so many different fillers on the market now, how to choose? There are 4 basic types: hyaluronic acids, poly-L-lactic acid, collagen and hydroxylapatite. Hyaluronic acids are the most widely used and have a tremendous amount of experience worldwide with no serious adverse events reported. Restylane was the first on the market and is still the leader in the US, and an important mainstay of our practice. It is a great all around filler and can be used in almost all areas. Juvederm was the next big hyaluronic acid filler to hit the market. The similarities between Restylane and Juvederm are much greater than the differences, in my opinion. Perlane has finally been FDA approved and adds a dimension to what we can offer you. It is a thicker filler and great for under eyes, nasolabial folds, marionette lines, lips and cheek augmentation as it provides tremendous volume and more longevity (usually 9-15 months). However, it isn’t good for areas of superficial wrinkles. Poly-L-lactic acid, Sculptra, is wonderful for overall volume loss and sagging, as well as the typical areas for fillers. These fillers provide a liquid lift (when achieved with hyaluronic acid, it may require more product) which can last as long as three years! Finally, hydroxylapatite, Radiesse, is also a robust filler for similar areas that Perlane is good for and similar longevity (however this filler should not be used in lips). Of course you don’t really have to know the differences as we will recommend what we think will be best for you at your consultation visit, but with so much press out there, many people want to understand the differences.

Pearl In May, we added the Pearl to our practice. This treatment device improves facial sun damage and aging changes (such as brown spots, mild wrinkles, etc) by gently removing the top layer of skin, much like our previous erbium laser peels. The Pearl treatments, however, require no post treatment topical bandages and result in faster healing time. With only 3-7days of redness and flaking, patients feel comfortable going about their activities with makeup by the third day post treatment. Still the only limiting factor for this device is that it cannot be used on very dark skin types. We will continue to use our Fraxel® SR1500 for those patients with darker skin to produce similar results to the Pearl. So as not to be too confusing, however, the Fraxel® SR1500 is still our best bet for scarring, abnormal pigment like melasma, and deeper problems in the skin.

Cosmelan® Depigmentation Treatment Cosmelan® Depigmentation Treatment is a safe and effective treatment peel that lightens hyper (dark) pigmentation associated with melasma. In addition, it brightens and smoothes the skin surface. It works by inhibiting tyrosinase, an enzyme that produces melanin. This treatment is safe for all skin types and can be used all year round. After the in-office treatment, an at home regimen will continue to improve the results.

VelaFirme VelaSmooth™ is an FDA approved medical device used to treat cellulite and contour body areas. ReFirme™ST is an FDA approved medical device used to tighten lax skin tissue and sculpt the skin. The combination of these treatment therapies, resulting in the VelaFirme™ allows for overall skin tightening and contouring, while reducing the appearance of cellulite. The VelaSmooth™ uses elo–s combination of bi-polar radiofrequency, infrared light, plus negative pressure (vacuum) and tissue manipulation. This combination of infrared and RF energies increase oxygen to the cells by heating the skin. This increases the metabolism of stored energy, there by shrinking the size of the fat chambers. Vacuum and specially designed rollers manipulate and smooth out the skin to facilitate safe and efficient delivery of the energy. Now with a recently developed smaller applicator, the VelaSmooth™ is capable of precisely targeting and treating smaller contoured areas such as the neck and arms or focusing on problem areas of the thighs, buttock, and hips. The ReFirme™ ST uses infrared and bi-polar radiofrequency energy to heat the dermal collagen tissue to cause contraction resulting in tighter skin. Thousands of patients have been treated worldwide with both therapies individually. The VelaFirme, the next wave, is combining the two methods together to yield faster reproducible results in fewer treatments. Contact us now for a consultation on how the VelaFirme can help you!

Folliculitis Many patients come to the office for treatment for this common skin disorder. Folliculitis is characterized by small red bumps in the hair follicles, and is often confused with acne when it occurs in the beard area or scalp. This condition can occur anywhere on the body, and is often the cause of those pesky bumps that women can get on their thighs after shaving. When mild, folliculitis may just be a few red bumps or papules, and when flared it can appear as an inflamed, red and itchy rash. Dipping your razor in alcohol between treatments is one way to help prevent you from spreading the bacteria to different parts of your skin. Glytone Serious Shave Cream, available at skinfo®, is can be helpful for reducing bouts of folliculitis. Persistent folliculitis can requires the prescription of a topical or even oral antibiotic. If you have been unable to control this on your own, please see a provider.

Acne and Rosacea For most people, warmer weather means more time spent outside, more sweating, and often more flares of their acne and rosacea. The occasional person has better control of their symptoms during warm seasons, but most people with acne or rosacea will need to make some changes to their regimen during the hot months of summer. Increased oil production can require stronger topical medications and cleansers. Toners, masks and more potent cleansers are often just what is needed to take care of the extra oil. People with rosacea can benefit from the year round use of a sunscreen that contains zinc and/or titanium, and that has an SPF of at least 30, but in the summer the role of a sunscreen becomes even morecritical. Acne patients sometimes haveincreased sun sensitivity due to their medications and should be using sun protection too. Make the Skinfo® Boutique your first stop for oil control during the summer but if breakouts are out of control, call your provider for advice.

Summer Skin Concerns The most common skin problems of summer are PMLE (aka sun poisoning), bug bites and poison ivy. Here are some tips to try to avoid these problems and to treat them when they do erupt:

PMLE (sun poisoning) is best avoided by using a physical block that contains zinc oxide and/or titanium dioxide. Regular over the counter sunscreens that claim to have UVA/UVB protection will not help to prevent this condition if these ingredients are not present, so read your labels! Wearing sun protective clothing (Coolibar® Quality Sun Protection clothing now available at skinfo®) is also helpful and if you do have a rash from the sun an over the counter cortisone cream will often help to calm down the reaction. Prescription products can be necessary with a severe eruption.

Bug Bites Use insect repellent to help prevent the bites from occurring, but if you do get a bug bite take anantihistamine (Claritin, Benadryl) and apply ice and hydrocortisone creamto relieve the itching. If you are VERY prone to a more intense reaction to the bite you may benefit from taking a daily antihistamine to help prevent the swelling and itching. Please refrain from applying sunscreens combined with insect repellant! We need to reapply sunscreen often and if insect repellents are applied too frequently they can cause toxicity in our nervous system.

Poison ivy or sumac If you come in contact with a “poisonous” plant besure to take a shower right away to get the rhus antigen (the chemical that is the allergen) off of your skin. If you think you got it on your clothes be sure to wash these items as well and gives animals a bath if they have been rolling in the plant. Usually you need a higher strength topical steroid than OTC products to get rid of the rash, but antihistamines, cortisone creams and compresses like Domeboro’s can help to soothe the skin.
