Acne Keloidalis Nuchae: Acne and Scarring of the Scalp

Acne Keloidalis Nuchae is a condition we don’t often hear about but has severe medical and cosmetic implications to those who suffer from it.

This is a chronic inflammatory condition (like acne) that typically affects the back of the scalp/posterior hairline [or beard area – this is called acne keloidalis barbae or pseudofolliculitis barbae] causing pimples and pustules around the hair follicles. With prolonged inflammation, the area can become swollen, painful and lead to firm scars called keloids resulting in focal but often permanent hair loss.

This condition is most common in men, especially those with coarse or curly hair. Short haircuts or close shaving of the hair is a large contributing factor but friction or irritation of the hairline can also play a role. Prompt and early treatment is imperative as once scar formation and hair loss occurs, they can be permanent. Early interventions include lifestyle changes, such as avoidance of close shaves/hair styles, and topical therapies including prescription antibiotics and exfoliants. These topicals help reduce inflamed papules and prevent formation of new lesions.
